Before You Book Your Appointment

We are credentialed with select insurance plans. Patients are responsible for verifying their coverage prior to booking the appointment. If we are not in-network with your insurance, or your plan does not cover services provided, cash pay options are available. Please note that in these cases, payment for services will be the patient’s responsibility, and due at the time of service.

New patient paperwork will be submitted via email. If patients are unable to access you can fill them out in person prior to your appointment!
